Kidney transplants have saved the lives of thousands of patients with end stage renal disease. In spite of its enormous benefit, transplantation nevertheless still has two major challenges: the risk of graft rejection and the need for lifelong immunosuppression. Because of graft failure related to immunological rejection, many patients must return to dialysis and/or undergo a second transplant. Chronic immunosuppression is required to prevent graft rejection but is itself associated with a host of serious and potentially life threatening complications. These include infections, malignancy, hypertension, diabetes, heart disease, and paradoxically recurrent kidney failure.
It has long been the dream of transplant scientists and physicians to discover a solution to the problem of graft rejection and the heavy burden of chronic immunosuppression. The kidney transplant team at UCLA developed a program to combine kidney and hematopoietic stem cell transplant to create a state of immune tolerance in which the patient’s immune system will not reject the transplanted kidney and there will be no need for ongoing immunosuppression. Based on previously established successful protocols, eight patients received human leukocyte antigen matched living kidney transplants. As one of the three largest kidney transplant programs in the United States and with a wealth of clinical and basic science expertise in transplantation, UCLA has been FDA approved to perform the Stem Cell and Kidney Transplant Surgery.
UCLA worked with top experts from Stanford and other medical institutions to train clinical technicians and surgeons on this process.
